If you are interested in purchasing a field day shirt please fill out this Google Form: https://forms.gle/w5hhjgCgVSC8HVh1A. Shirts cost $10.00. Payment is due Tuesday, June 7th: -Paypal: @cookwisshsa2021 -Venmo: @cookwisshsaSend -cash or check (check payable to Cook-Wiss HSA)  Please complete the District-Wide Parent/Guardian Survey by June 10th here. Survey results are used by schools to: -Design annual plans and secure funding for faculty, staff and materials. -Address challenges families may face with communication from schools, student attendance, and quality of instruction. -Ramp up community resources and services that children and families need. -Celebrate progress and success!
